MISSION

Champions Community Foundation exists to empower young adults with physical challenges to reach their potential for independence and belonging.

SUMMARY

In the Atlanta Metro area, conservative estimates show there are over 11,000 young adults between the ages of 18 and 35 with physical disabilities due to cerebral palsy, spina bifida, muscular dystrophy and physical injury; estimates indicate there are over 630,000 nationally. The needs of this existing population are great. With each successive year, a new group of young adults with disabilities “ages out” of high school, finding themselves increasingly isolated at home, dependent upon aging parents for personal care and support. To help address these challenges, the founders of Champions Community Foundation created the Titan Program in 2009 by implementing a broad range of athletic and social programs. The program grew from 6 to engage a community of 90+ physically challenged young adults, known as the Titan Community. The program is designed to build community and friendships. The Titans also serve as community ambassadors, fostering understanding and education about physical disabilities.

In 2014, Champions Community Foundation broadened its focus by pursuing the development of Champions Place, a uniquely designed, safe, accessible and affordable shared living community where these young adults would live independent and productive lives. After opening in October of 2020, 14 physically challenged young adults now call Champions Place “home.” Through the use of the latest assistive technology such as automated doors, voice activated lights, fans, TVs, shades and more, the residents are learning to become more self-reliant. And, due to its large common area and backyard, Champions Place is now the new social hub for the Titan Community hosting events such as cookouts, tailgates and holiday parties.

The Champions Community Foundation’s Strategic Plan outlined in the following pages seeks to enhance the quality of its services, expand service provision to more young adults, and increase its service portfolio sustainably, while simultaneously developing new tools and best practices to support parents raising their physically challenged young adults at home.

GOAL #1

FURTHER DEVELOP
THE CHAMPIONS PLACE MODEL

Objective 1.1: Develop a more sustainable, long term caregiving model at Champions Place that is no longer reliant on the involvement of the resident’s parents by June 30, 2025

Activity 1.1.1: Create a working group to develop a sustainability strategy for caregiving after parents have passed. The group will be created by December 31, 2022. 

Activity 1.1.2: Research existing resources and practices in other states by June 30, 2023. 

Activity 1.1.3: Develop an internal strategy to minimize negative impact on caregiving from parents passing by June 30, 2024. 

Activity 1.1.4 Work in collaboration with the Director of Medicaid and State Legislators to develop a solution by December 31, 2024.

Activity 1.1.5: Develop an implementable strategy to minimize negative impact on caregiving from parents passing by June 30, 2025. 

Objective 1.2: Document the Champions Place operational model by June 30, 2024.

Activity 1.2.1: Prepare a repository to store operating procedures, along with a master list of operational procedures to be developed. Create a template and instructions for developing the standard operating procedures. Train staff who will be creating standard operating procedures by May 31, 2023.

Activity 1.2.2: Complete and track to completion the master list of operating procedures by December 31, 2023. 

Activity 1.2.3: Create and implement a cycle for standard operating procedure review and update by June 30, 2024. 

Objective 1.3: Actively engage and work alongside 5 qualified organizations by sharing our best practices to help them develop a Champions Place in their local community resulting in at least 2 organizations being committed to move forward by June 30, 2025. 

Activity 1.3.1: Leveraging the standard operating procedure created in Objective 1.2.2, develop a portfolio of best practices to share and educate visiting groups by September 30, 2023. 

Activity 1.3.2: Determine whether to profit, recover cost or donate these services by December 31, 2023. 

Activity 1.3.3: Proactively identify interested organizations by January 31, 2025. 

Activity 1.3.4: Prioritize our efforts with qualified organizations with a goal that at least 2 commit to break ground by June 30, 2025. 

Objective 1.4: Explore alternative replication or expansion models for Champions Place

Activity 1.4.1: Create a working group to research alternative models and best practices by January 1, 2023.

Activity 1.4.2: Engage research team and identify other potential models by January 30, 2024.

Activity 1.4.3: Present findings to the board of directors by June 30, 2024.

Activity 1.4.4: Conduct site visits to leading programs to continue research by March 30, 2025. 

Activity 1.4.5: Present final recommendations to board and begin applying lessons learned in planning for Champions Place II program planning by June 30, 2025.

Objective 2.1: Provide a robust schedule of events for the Titan Community in order to increase participation by 15% by June 30, 2023.

Activity 2.1.1: Plan bi-monthly scrimmage schedules 30 days prior to each quarter starting by August 31, 2022 and ongoing each year until June 30, 2025.

Activity 2.1.2: Increase participation by planning social events every other month at least 2 months in advance, leveraging Champion’s Place where appropriate, starting August 31, 2022 and ongoing each year until June 30, 2025.

Activity 2.1.3: Develop and implement a “New Titan Roadmap” by September 30, 2023 to ensure each Titan is warmly welcomed, on-boarded and stays connected to the Titan Community.

PROVIDE COMMUNITY
AND FRIENDSHIPS 

Objective 2.2: Create a virtual community of 1,500 young adults with physical challenges to increase social connectedness and offer support for practical life challenges by June 30, 2025. 

Activity 2.2.1: Create a working group to research virtual programming structures by December 31, 2022.

Activity 2.2.2: Evaluate what services can be reasonably offered in a virtual setting. Research and present recommendations for the structure of virtual programming to Executive Director by July 31, 2023. 

Activity 2.2.3: Launch a pilot as an initial phase of the program to test participation and it's effectiveness in increasing feelings of belonging and community in young adults with physical challenges by December 31, 2023. 

Activity 2.2.4: Based on participation and feedback from the pilot phase, present the team’s recommendation for creating a staffing structure to provide more robust programming and social services in a virtual setting. Determine staff needed and hire by June 30, 2024. 

Activity 2.2.5: Improve virtual and social media presence of Champions Community Foundation to recruit 1,500 young adults with physical challenges as users in virtual programming by June 2025.

BY LEVERAGING WHAT WEHAVE LEARNED 

Objective 3.1: Expand Champions Place@Home services to 250 young adults with physical challenges by June 30, 2026. 

Activity 3.1.1: Survey Titan Community to identify 24 young adults with physical challenges who are interested in applying for Champions Place@Home by December 30, 2022.

Activity 3.1.2: Execute a total of 18 individual plans for service by June 30, 2023.

Activity 3.1.3: Evaluate the effectiveness of the use of assistive technology using the Champions Place@Home model via survey by December 30, 2023. 

Activity 3.1.4: Use survey data to update programming to be more effective for young adults with physical challenges by March 31, 2024 and annually thereafter. 

Activity 3.1.5: Define scope and method of post-installation support for Champions Place@Home by June 30, 2024.

Activity 3.1.6: Develop a strategic partnership to enable us to increase our reach and scale of Champions Place@Home installations by June 30, 2024. 

Activity 3.1.7: Expand the offering to physically challenged young adults outside the Titan community with the goal of reaching 250 Champions Place@Home installations by June 30, 2026.

Objective 3.2: Leverage the knowledge and experience of partners, parents and staff to develop and offer 3 to 4 informational sessions per year to 80 Titan parents to help them support their physically challenged sons and daughters who have or will be transitioning out of high school by June 30, 2024.

Activity 3.2.1: Create a working group of stakeholders to focus on educating parents by July 31, 2023

Activity 3.2.2: Gather feedback from parents and staff to understand the skills and challenges of parents of young adults with physical challenges through a digital survey and 1 listening sessions by August 31, 2023.

Activity 3.2.3: Identify 3 or 4 topics that represent the areas of greatest need for resources and support by January 1, 2024. 

Activity 3.2.4: Conduct 3 to 4 training sessions on a quarterly basis during 2024 with parents of young adults with physical challenges by June 30, 2024.  

Activity 3.2.5: Survey parents to gain feedback on effectiveness of sessions and additional areas of interest by March 31, 2025.


GOAL #4

ESTABLISH SUSTAINABILITY FOR OPERATIONS 

Objective 4.1: Increase the long-term sustainability of leadership of Champions Community Foundation for the future through succession planning and strengthening the board of directors by July 1, 2024.

Activity 4.1.1: Create a written succession plan for leadership transition that is approved by the board of directors by March 31, 2023.

Activity 4.1.2: Evaluate the board of directors to ensure biographical and skill diversity by February 28, 2023. 

Activity 4.1.3: Recruit 5-7 new board members to fill diversity gaps by September 30, 2024. 

Activity 4.1.4: Conduct board training 2 times per year starting October 1, 2024. 

Objective 4.2: Increase the sustainability of the organization by expanding its fundraising capabilities in the areas of annual fund contributions and grant awards over 3 years. 

Activity 4.2.1: Create a written development plan by February 28, 2023.

Activity 4.2.2: Realize a 10% increase in annual fund contributions from private donors each year for 3 years. 

Activity 4.2.3: Realize a 10% increase in grant awards from institutional donors each year for 3 years. 

Activity 4.2.4: Develop and launch an endowment campaign to increase its value to $2.5M by June 30, 2025.
